An HC2 certificate is a blue, red and white document which entitles you to free minor treatment from doctors and dentists, for example tablets and creams. It also entitles you to discounts on other treatments, for example glasses. An HC2 certificate also enables you to claim travel expenses to and from hospital for specific treatment. You are entitled to an HC2 certificate if you receive a low income, NASS support or other benefits. You can apply for an HC2 certificate by completing an HC1 form. You can pick up an HC1 form from a Jobcentre Plus Office or NHS hospital. Your doctor, dentist or optician may have one too. You can also request a form by telephoning the Health Literature line. |
